Summary

Community pharmacy discharge medicines services have been shown to have a positive impact on patient care. Remote consultations, as well as face-to-face, can be a valuable tool in reducing errors, improving safety and enhancing care post-discharge. Referrals from schemes such as the DMS also create opportunities for communication with other care sectors, improving links with community pharmacy.

Reflection exercise

Think about a recent DMS consultation where you identified a discrepancy between the hospital medication list and your own information from your PMR system.

  • Was your DMS questioning approach helpful in exploring the discrepancy in a non-judgmental way?
  • How did you communicate with other sectors of care to obtain timely information? Could this be improved?
  • How did you reassure the patient about their medicines? Did it work?
