Community pharmacy discharge medicines services have been shown to have a positive impact on patient care. Remote consultations, as well as face-to-face, can be a valuable tool in reducing errors, improving safety and enhancing care post-discharge. Referrals from schemes such as the DMS also create opportunities for communication with other care sectors, improving links with community pharmacy.
